Flex Reporting

Flex will provide each partner, as applicable, with a report with the following data (the “Data”) pursuant to the terms of the agreement between Flex and such partner (the “Agreement”). The Agreement will govern this data in all respects.The Data shall be fully de-identified, aggregated and anonymous in accordance with applicable privacy laws and industry standards. Partner certifies that it will not attempt to re-identify any individual whose data is included in the Data and will not permit any third party to do so.

1. Enrollment status in the Flex Services (yes/no);
2. Date of enrollment in the Flex Services;
3. Date and amount of successful payment to partner using the Flex Services; and
4. Date of cancellation of the Flex Services (if applicable).
